How it was found: clicking through every page in a nested Hyprland session, 2026-10-09.

Confidence: high.

Bug: a bind with no mods of its own uses default_mods (SUPER), but the keycap button rendered only the key. "Open browser" read as plain B, which looks like typing B triggers it. Shortcut search also did not match "super".

Fix: on fix/v2-review-gaps (commit 76dd6ca). Keycaps and search use the effective modifiers. "Default mods" became a "Main modifier key" picker, and the "layout" and "exit" actions got plain names.
