gitignore: stop tracking graphify-out/ tool cache

graphify's local knowledge-graph output was never gitignored, so its
generated cache/graph files (75 tracked entries) were showing up as
dirty noise on every run. Add graphify-out/ to .gitignore and untrack
the existing files.
