Part of BUG-EP1, implements breadboard issue BUG-21 (promote manual reports and requests with status sync). Stacked on the BUG-20 branch.

The existing promote, growth comment, fixed detection and regression flow now covers manual and request clusters: evidence carries title and want text, issue bodies name the kind plainly, labels use the kind, and the triage sample view renders title, wants and why. Developer control is unchanged: only promoted groups get issues, board stays authoritative.

Verified: cargo test (all suites green, 1 new test), cargo clippy with -D warnings clean, cargo fmt clean.

Extend the breadboard promotion flow to manual and request clusters:
evidence blocks carry the filed title and want text, issue bodies say
how each kind was found and name it plainly, and the triage sample
view renders title, wants and why. Growth comments, fixed detection
and regression keep working unchanged for the new kinds.

Address review on the manual/request promotion slice:
- tracked_issues now includes fixed and dismissed groups, so their board
  issue keeps being followed instead of freezing at tracked state
- sync_issue_states re-derives the public state when a done issue moves
  back out of done: a board-driven fix (no recorded version) returns to
  promoted, while an admin mark-fixed (with a version) is left alone
- tests: done then reopen for manual and request clusters, plus failed
  create retry and double-promote refusal for the new kinds

Address review on the promotion slice:
- add a board_fixed flag: a fix that came from the board reaching done is
  re-derived when the board reopens, while an admin mark-fixed (which sets a
  version) is left alone, so the reopen no longer keys off fixed_in (which
  BUG-8 now populates)
- tracked_issues includes deferred clusters so a deferred promoted cluster
  keeps being followed
- tests: an admin fix is not reopened by the board
