web: polish pass (favicon, avatars, delete confirm, transitions, empty states)

Details

- add a favicon + meta description (index.html had neither)
- render the signed-in user's real Forgejo avatar in the header and on
  cards assigned to you, instead of plain text / a bare glyph
- require a second click to delete an issue instead of firing instantly
- add small, consistent open/close transitions to the command palette,
  notification panel, issue drawer, and toast
- empty board columns show a dim placeholder instead of dead space; cards
  get a subtle hover lift instead of just a border-color change
